IMPORTANT HEALTH COVERAGE TAX DOCUMENTS
The Paperwork Burden Reduction Act of 2024 provides that employers are not required automatically to distribute Forms 1095-B and 1095-C to full-time employees and other employees enrolled in the health plan. You may receive a copy of the Form 1095 by submitting a written request to: Renee Seacat at rseacat@bgcs.k12.in.us, 317-788-4481 x1046 or by written request at Beech Grove City Schools, Attn: Payroll Department, 5334 Hornet Ave., Beech Grove, IN 46107.
